• State License 1 Verification
  • Job Details
  • 2 years required
  • Cert required: NBSTSA
  • Float requirement: possibly to main hospital
  • Must be a CSFA
  • Shift: 5 days a week 0700-1530
  • Call Requirements: This call is strictly for CT surgery. 2-3 evenings a week (1530-0700) then it will be every other weekend 0700a-0700a Saturday and 0700a-0700a Sunday.
    • Job Requirements
    • Valid Driver's License or Passport accepted. Must be clear copy.
    • Online Verification of License to include expiration date and any disciplinary actions (within 30 days before start date). Must be through primary source. IN.gov.LICENSING: Task the Program Manager for approvals on license issues.
      • Schedule Information
      • Shift: 5 days a week 0700-1530
      • Call Requirements: 2-3 evenings a week (1530-0700) then it will be every other weekend 0700a-0700a Saturday and 0700a-0700a Sunday.
        • Unit Specific Information
